The Solid Organ Transplant Clinical Pharmacy Specialist will provide expert pharmaceutical care to pediatric patients undergoing renal or cardiac transplantation, as well as those with complex nephrological conditions (including dialysis) in both ambulatory and inpatient settings. This role requires a deep understanding of complex immunomodulatory, transplant, and renal pharmacotherapy, along with the ability to develop individualized treatment plans.
Schedule: Monday-Friday 8:00am-4:30pm. Note: The Specialist serves as the primary point of contact for the Renal Transplant team outside of normal business hours.
Responsibilities
Patient Care:
Develop and implement comprehensive medication regimens for patients with renal transplant, cardiac transplant, or complex nephrological diseases.
Monitor patient response to therapy and aggressively adjust medication regimens as needed, including therapeutic drug monitoring for complex immunosuppressants.
Provide specialized medication counseling and education to transplant recipients, pediatric patients, and their caregivers.
Collaborate closely with multidisciplinary healthcare providers across the nephrology, cardiology, and transplant teams to optimize patient outcomes.
Clinical Services:
Participate actively in pre-transplant evaluations and comprehensive post-transplant care for both heart and kidney recipients.
Manage complex medication-related adverse events, opportunistic infection prophylaxis, and critical drug interactions.
Provide expert consultative services to medical, nursing, and surgical staff regarding solid organ transplant and complex renal pharmacotherapy (including continuous renal replacement therapy and maintenance dialysis).
Research and Education:
Stay current with the latest clinical practice guidelines and advancements in solid organ transplantation and pediatric nephrology.
Contribute to quality improvement and research projects related to renal and cardiac transplantation.
Participate in and lead educational programs for healthcare providers, pharmacy residents, and patients and their caregivers.
Requirements
Qualifications
- PharmD from an ACPE accredited school of pharmacy is required.
- Completion of a PGY1 and an applicable PGY2 specialty residency (Solid Organ Transplant or Pediatric Pharmacy preferred), or a minimum of 5 years of equivalent clinical pharmacy experience is required.
- Texas Pharmacist license (or eligible).
- Texas Preceptor license required (must be obtained within 90 days of hire).
- CPR - BLS required.
- Board of Pharmacy Specialties (BPS) certification required within 1 year of employment (BCPS, BCPPS, or BCTXP).
Preferred Skills: Prior experience related to renal/cardiac physiology, transplant immunology, immunosuppressive pharmacology, and management of dialysis populations is strongly preferred.
