Job Summary | CW PREP is an innovative child welfare workforce preparation academic program. The goal of CW-PREP is profoundly simple: Foster an unparalleled pathway for recruiting, supporting, and retaining a vibrant, dynamic public child welfare workforce. Against that backdrop, CW PREP is steadfastly committed to the wellbeing academic and otherwise of students participating in CW PREP. As such, the College of Social Work (CoSW) is searching for an Academic Case Manager (ACM). Utilizing a strengths-based approach, the ACM will deploy contemporary transdisciplinary case management models to assist students in traversing dynamic service and support continuums across CW PREP partner institutions; provide direct academic case management (non-clinical) services; and conceptualize innovative ways to ensure CW PREP participant success. In conjunction with the CW PREP program team and the CW PREP Oversight Council, the ACM will be responsible for the following duties: Wholistic Service Promulgation • Provide psycho-social (non-clinical), academic, professional, and life-skills supports to participants • Lead CW PREP self-care and wellness initiatives • Coordinate transitional initiatives aimed at workforce preparation • Maintain coordinated service and contact records using case management software Academic and Educational Service Referral, Navigation & Coordination • Serve as primary contact for referral management and response program staff • Conduct assessments that identify appropriate service tracks for program participants • Work with students to navigate intra/inter service units within and between institutions • Facilitate program service team meetings to ensure uniformed service provision Systemic Program Conceptualization • Initiate ongoing cohort assessments to inform service needs and responses offered by CW PREP • Undertake activities related to planning, implementing, and evaluating service provision • Assess systemic service needs/deficits and make recommendations for improvements • Initiate and maintain CW PREP service network assessments and referral processes Academic Constituent Outreach • Work with program participants to build capacity and address access barriers to academic resources • Assist participants who experience academic disruptions • Create and disseminate information about resources and services • Foster collaborative relationships that provide opportunities for program participants |
